This individualized nutritional therapy program is designed to help you reclaim your health through sustainable nutrition and lifestyle changes.
Over the course of three months we will work to identify your unique health challenges, set achievable goals, and implement targeted strategies tailored to your unique needs.

What’s included in 12-Weeks to Wellness:

This includes an Initial Interview Questionnaire, Nutritional Assessment Questionnaire (NAQ), Symptom Burden Graph, 7-Day Food & Mood Journal Audit, and 90 min Initial Interview Call where we will go over all of the information you have provided, and ask more questions in order to serve you well and provide the best recommendations possible.

The NAQ Symptom Burden Graph is a visual representation of the results from the Nutritional Assessment Questionnaire (NAQ). It uses a graph to display the level of symptom burden across different nutritional foundations and body systems, based on the answers provided in the NAQ. This graph helps me as your practitioner understand which areas of health are most impacted by symptoms and where support is needed. You will receive your results at Week 1 and again at Week 12.

During the Initial Assessment, I will send you a journal to complete prior to the Recommendations Call. This journal is very detailed and includes not only the food you are eating throughout the day, but also the reactions and way you are feeling after each meal. I also provide you a chart to track your bowel movements as this can be extremely helpful in understanding the root cause of your issues.

After gathering and analyzing all of the information from the Initial Assessment, we will have a 30 min Recommendations Call to go over my personalized recommendations for you based on the following: diet, digestion system, hydration, mineral balance, fatty acid status, blood sugar regulation, sleep, stress, movement, and environmental factors like toxins found in everyday household items.

These 20-30 min calls will be a way for us to touch base regarding my recommendations and how you’re doing with implementing them. We will discuss any changes or adjustments that may need to be made. My goal is not to overwhelm you, but provide you practical steps you can take to achieve your wellness goals.

All clients receive 20% off top-quality supplements from my Fullscript dispensary. However, a food first approach is something I prioritize in my program. It’s important to get nutrients primarily from whole, real foods before turning to supplements. It’s rooted in the belief that the body functions best when it receives its nourishment in the way nature intended—through nutrient-dense, properly prepared foods. Supplements may be recommended when there’s a confirmed deficiency, the body needs temporary support, or diet alone isn’t sufficient due to absorption issues, illness, or life stage needs.

This includes a Nutritional Assessment Questionnaire (NAQ), Symptom Burden Graph, and 7-Day Food & Mood Journal Audit. After this information is gathered for a second time, we will evaluate the progress you’ve made over the last 12-weeks. This is our last and final call together in the program and it is always bittersweet. After this call, I will send you a Certification of Completion via email. In between our biweekly coaching calls, you will have the opportunity to send me unlimited texts and emails. This is something I encourage clients to take advantage of and what makes my program unique from other medical providers. There will be no rushing you “out of my office.” The level of care you receive from my program will be dependent on your dedication and level of commitment to your plan.

The orange line represents a baseline dysfunction within various areas of the body during the Initial Assessment (week 1). The blue line represents the progress made after working together (week 12).
