12-Weeks to Wellness 1:1 Health Coaching Program is a 100% virtual, individualized, comprehensive approach designed to help you reclaim your health through sustainable lifestyle changes. Over the course of three months, we will work closely to identify your unique health challenges, set achievable goals, and implement targeted strategies tailored to your needs.

This program is ideal for individuals seeking a structured, evidence-backed path to improve energy, enhance digestion, balance hormones, reduce inflammation, and achieve optimal health. By the end of 12-weeks, you will have established habits and gained the knowledge to maintain lasting health transformations.

What’s included in the 12-Weeks to Wellness Program:

  • This includes an Initial Interview Questionnaire, Nutritional Assessment Questionnaire (NAQ), Symptom Burden Graph, 7-Day Food & Mood Journal Audit, and 90 min Initial Interview Call where we will go over all of the information you have provided, and ask more questions in order to serve you well and provide the best recommendations possible.

  • During the Initial Assessment, I will send you a journal to complete prior to the Recommendations Call. This journal is very detailed and includes not only the food you are eating throughout the day, but also the reactions and way you are feeling after each meal. I also provide you a chart to track your bowel movements as this can be extremely helpful in understanding the root cause of your issues.

  • After gathering and analyzing all of the information from the Initial Assessment, we will have a 30 min Recommendations Call to go over my personalized recommendations for you based on the following: diet, digestion system, hydration, mineral balance, fatty acid status, blood sugar regulation, sleep, stress, movement, and environmental factors like toxins found in everyday household items.

  • These 20-30 min calls will be a way for us to touch base regarding my recommendations and how you’re doing with implementing them. We will discuss any changes or adjustments that may need to be made. My goal is not to overwhelm you, but provide you practical steps you can take to achieve your wellness goals.

  • All clients receive 20% off top-quality supplements from my Fullscript dispensary. However, a food first approach is something I prioritize in my program. It’s important to get nutrients primarily from whole, real foods before turning to supplements. It’s rooted in the belief that the body functions best when it receives its nourishment in the way nature intended—through nutrient-dense, properly prepared foods. Supplements may be recommended when there’s a confirmed deficiency, the body needs temporary support, or diet alone isn’t sufficient due to absorption issues, illness, or life stage needs.

  • This includes a Nutritional Assessment Questionnaire (NAQ), Symptom Burden Graph, and 7-Day Food & Mood Journal Audit. After this information is gathered for a second time, we will evaluate the progress you’ve made over the last 12-weeks. This is our last and final call together in the program and it is always bittersweet. After this call, I will send you a Certification of Completion via email.   • In between our biweekly coaching calls, you will have the opportunity to send me unlimited texts and emails. This is something I encourage clients to take advantage of and what makes my program unique from other medical providers. There will be no rushing you “out of my office.” The level of care you receive from my program will be dependent on your dedication and level of commitment to your plan.

  • Discover a personalized nutrition plan made for your unique body, lifestyle, and health goals. Unlike one-size-fits-all diets, this approach considers your genes, metabolism, gut health, and environment. This is a program I wish I had 15 years ago when I received my initial PCOS diagnosis. It probably would have saved me YEARS of trial and error. My hope is that whether you are just starting out your wellness journey, or if you’ve been on a journey for awhile, this program can be a beam of light to you!

  • These resources will be emailed to you along our 12-week journey together. If there is a particular resource you are interested in, don’t be afraid to ask!
